The majority of hospice care is provided in the home setting with a focus on pain and symptoms management, coordination of care and communication. Under home hospice, a family member or close friend serves as your primary caregiver and is responsible for providing or overseeing most of your care. Our care team of professionals and volunteers, including doctors, nurses, home health aides, spiritual counselors and volunteers, works with you and your caregivers to develop a plan tailored to meet the unique needs of your situation.

Members of your TMC Hospice care team will meet regularly with you and your caregivers to evaluate your needs, provide medical services and offer assistance with personal care. Additionally, TMC Hospice's Heart Connections program can help you stay at home where you will be most comfortable in dealing with a progressing cardiac illness, such as congestive heart failure.
