TMC One is Your Medical Home

TMC One provides our patients with a medical home. Our aim is to work as partners with our patients to provide patient-centered services that are comprehensive, timely, convenient, safe and satisfying. 

What do we mean by patient-centered? We mean our practice is built around you, the patient, and your needs. Picture yourself and your physician as the hub of a wheel, in partnership to maximize your health and well-being. The spokes, then, are your specialists, your hospital, your insurance company, your pharmacy and all your other healthcare and medical needs. 

At TMC One we want to make sure you get the care you need, when you need it. What this means to you: 

Available 24/7

We are available around the clock. If you are experiencing an emergency medical health crisis, call 911 immediately. But for urgent concerns, call your doctor’s office. After normal business hours, you can still call. The practitioner on call will evaluate your health circumstances and direct you to the care you need. 


The more you understand your health and medical needs, and the more we understand you, the more quickly we can make sure you're on the right path to care. Please be honest with your providers about your medical and health information. Let us know how you feel and what you think. If you are worried about a drug or treatment, if you are using complementary treatments, or if there is anything that has the potential to affect your health, please tell your doctor. By working together, we want to empower you to be an active partner in your own care. 

During your appointments, our physicians and staff are happy to answer all your questions so you fully understand your current health, your options and your plan of care. And your doctor also needs to know if you have any concerns with specialty care or other services you receive outside of TMC One. Your primary care physician wants to understand you as a whole person. 


For those with a chronic condition, complex medical needs or a multitude of medical issues, managing your health can become complicated and frustrating. Many of our patients are under the care of a number of specialists, taking a multitude of medications and undergoing multiple medical therapies. We can help you sort it out. Coordination of Care

As your medical home, TMC One will work with you to ensure your care is coordinated among specialists, hospitals, home health agencies and nursing homes. It helps us if you make sure reports from your visits to other doctors or facilities are sent to your primary care physician at TMC One.
